The Geisinger 65 Forward Health Centers are devoted to promoting health and leisure activities for older adults. In a recent event, the Buckhorn group gathered at the spacious Bloomsburg Town Park to participate in a session that commenced with fitness education. They then engaged in cognitive exercises that combined physical activity with on-the-spot problem-solving challenges, and also enjoyed games such as ladder ball and cornhole.
Concurrently, the State College group took their walking session to the picturesque campus of the PA Military Museum in Boalsburg. Following their stroll, they indulged in refreshing smoothies at a nearby cafĂ©. Additionally, the Scranton group engaged in craft activities involving patriotic garlands and participated in a “Wheel of Fitness” exercise class.
The 65 Forward Health Centers provide specialized primary care for Geisinger Gold Medicare Advantage members. This encompasses extended appointments to assist in managing chronic conditions, as well as on-site laboratory and diagnostic imaging services. In addition, they offer top-notch equipment, exercise classes, and wellness coordinators to aid members in achieving their fitness objectives. Furthermore, they address senior isolation by offering social and educational group activities.
Research indicates that isolation and loneliness are prevalent among individuals aged 65 and above and can adversely affect their physical and mental well-being. Maintaining a vibrant social life is essential for their overall health.
A comparison of 65 Forward patients with a similar demographic receiving care in a traditional primary care model revealed noteworthy results. 65 Forward patients experienced approximately 15% fewer hospital admissions and about 40% fewer emergency department visits.
